Are You Providing Nutrition Services Under the NYS 1115 Medicaid Waiver?
Join a Technical Assistance Group!

Part of the mission of the NYS Food as Medicine Coalition is to enhance communication and collaboration between stakeholders. We have heard from FAM stakeholders across the state that there is a need to have space to share best practices, issues, and drive discussion to build even more best practices. In the spirit of our mission and to serve you, our members and stakeholders, we are offering monthly technical assistance groups based on type of service provided. Please register for the group that best matches your service.

Food Suppliers and FAM Providers

Food Supplier Survey

Are you a B2B retail/wholesale/private food supplier that sells food to FAM programs? Add your organization to the NYS Food Supplier Map so FAM programs looking to purchase food can find you and connect.

FAM Program Survey

Are you a direct-to-consumer (B2C) program that offers FAM services to individuals?  Add your information to the FAM Program Map so care providers who are looking to refer their patients or community members for FAM services can find your program. All FAM programs are welcome to submit, regardless of funding source.
